Maple Health Direct Primary Care (DPC) offers an increasingly popular healthcare model that bypasses traditional health insurance and offers immediate access to quality clinicians at an affordable price.

Care plans range from $70 to $120 per month and are available for individuals, families and businesses. Members enjoy unlimited access to priority care and personalized attention from one clinician who takes the time necessary to be your personal healthcare advocate. One consistent monthly cost for care allows members to budget healthcare expenses just like any other household payment.

Clinicians at Maple Health DPC offer everything expected from a primary care physician for patients from infancy to old age. Just as with traditional care, clinicians consult specialists when necessary.


Maple Health DPC boasts an in-office pharmacy and lab as a convenience and savings. Between Dr. Berry, physician assistants Courtney Kozek, PA-C, and Jordan Smith, PA-C, Director of Business Operations, Vanessa Padilla and Administrative Assistant, Erin Jelenic, Maple Health provides continuity of care from the first phone call through visits, treatments and beyond. Your first visit to Maple Health includes a complete history and physical examination, including bloodwork, a review of results, and a conversation about expectations and health goals. 

Capping their individual patient loads at 600 allows Maple Health DPC clinicians to build availability into their schedules and allot one full hour for annual physicals and thirty minutes for follow up appointments. In comparison, a typical primary care doctor sees 2,000 to 3,000 patients.


Business Membership

For businesses, Maple Health DPC is a low-cost way for companies to offer health care to employees and the cost for care is tax deductible. Employees’ increased access to health care results in a healthier workforce and a decrease in cost to employers.
