In short-term rehabilitation, long-term care and outpatient therapy, the comprehensive range of therapy services at The Laurels of Chagrin Falls provides a safe transition back home and a path for patients to achieve their goals.

So much of what our physical, speech and occupational therapists do on a daily basis has changed over the years.

When I first started in healthcare in 1999, the focus was limited to the immediate needs and to get a patient back home. Today, we direct our approach toward long-term goals, how the patient can care for themselves and thrive long after they return home.

When a patient first comes to us, for a stay or as an outpatient, we work together to perform an assessment and come up with a Pathway to Home plan of care. Then we conference with the family and pinpoint any barriers to a smooth transition. From the very beginning, we talk about when they will be discharged and what will happen at home. The goal of our therapy is functional based. Instead of exercise equipment, like a treadmill, we create obstacle encounters that could happen in a home. For instance, how to manage a laundry basket with a walker. We teach them how to handle these events in a safe way.

We work with patients of all ages and our therapists are extensively trained to work with the MRDD population, interacting with their group home settings or the workplace.

We are excited to announce that we are offering outpatient therapy services, as well as, in-home therapy services for local residents.
